The immunocompromised patient and transfusion.

چکیده
Immunocompromised patients are usually seriously ill and many such patients, especially those undergoing stem cell transplantation, have prolonged periods of pancytopenia and consequently, heavy transfusion requirements. All transfusions are potentially hazardous but transfusions to immunocompromised patients cause additional problems, which may be immunological or infectious. This review describes these special problems and ways to alleviate them. It should be useful to anyone treating immunocompromised patients particularly specialists in haematology, transfusion medicine, infectious diseases, oncology, transplant surgery, anaesthesia, and neonatology. DiVerent parts of the immune system— either non-specific (phagocytes, complement, etc) or specific immunity (cellular or humoral) or combinations thereof may be aVected. Patients with pure B cell immunodeficiency have few transfusion related problems. Both hereditary and acquired defects of the immune system occur (table 1). Inherited defects requiring transfusions are rare while acquired causes are relatively common. Neonates weighing less than 1200 g are physiologically immunocompromised.

Late onset post-transfusion hepatitis E developing during chemotherapy for acute promyelocytic leukemia.
We herein report the case of a leukemia patient who developed hepatitis E seven months after undergoing a transfusion with contaminated blood products. The latency period in this case was significantly longer than that of typical hepatitis E. Recently, chronic infection with hepatitis E virus (HEV) genotype 3 has been reported in immunocompromised patients. There is a possibility that our patie...

We report the first case of invasive subcutaneous infection caused by this fungus in a 78-year-old female from Damghan, Iran, with diabetes and chronic renal failure. A biopsy provided an initial diagnosis of the mold in tissue. Multiple positive fungal cultures which were obtained from the biopsie...

Materials and Methods: In this study, we report the first case with Rhnull, a blood group phenotype in southeast Iran, which was diagnosed during pretransf...
متن کاملMeta-analysis of cytomegalovirus seroprevalence in volunteer blood donors and healthy subjects in Iran from 1992 to 2013
Objective(s):Human cytomegalovirus (CMV), a double-strand DNA herpesvirus, can be transmitted via blood transfusion which is especially important for immunocompromised recipients and can cause a fatal infection. CMV seroprevalence in Iran was studied on blood donors, healthy subjects, and some patients. Highly variable rates were detected. The purpose of this study was to review CMV seroprevale...
